The Psychosocial Assessment Tool © (PAT) is a brief parent/caregiver-report screener of family psychosocial risk for families of children with pediatric health conditions. The PAT is copyrighted and is distributed by the Center for Healthcare Delivery Science (CHDS) at the Nemours Foundation (Nemours / Alfred I. duPont Hospital for Children). It may not be used or modified in any form without prior written permission from CHDS. The PAT has a yearly license fee of $350.
